Terms and conditions for Military & First Responders-Unlimited
There are no initiation fees nor multi-month contracts. Card on file is auto-billed on a monthly basis. Cancellation policy is 30 days’ advance notice from your billing date.
This purchaser agrees to purchase and begin services for Military & First Responders-Unlimited and running for thirty (30) days from the effective date.
PHYSICAL LOCATION:
This facility where services will be rendered is CrossFit MVA, located at 1712 Mount Vernon Avenue, Alexandria, VA 22301
CONTRACT TERM:
The term of this contract is in force for thirty (30) days from the Contract Start Date: DATE_______. This contract automatically renews at the end of the thirty (30) days for subsequent months, unless cancelled by notification by the policies outlined in the "Buyers Right To Cancel" section below. Contract detail summary:
- No initiation fee;
- No more than one (1) month’s service fees are purchased every thirty (30) days;
INITIATION FEE:
The initiation fees associated with this contract is $0. This purchaser agrees to auto-renew their access on a monthly basis to collect no more than one month’s dues at a time.
SERVICE FEE:
The Purchaser elects this service Military & First Responders-Unlimited and does not come with an initiation fee, and the purchaser has a right to cancel.
RIGHT TO CANCEL
If you, the purchaser, wishes to cancel this contract, you may cancel by making or delivering written notice to this establishment. The notice must say that you do not wish to be bound by the contract and must be delivered or mailed before midnight of the third business day after you sign this contract. This notice must be delivered or mailed to: CrossFit MVA, 1712c Mount Vernon Avenue, Alexandria VA 22301. If canceled within three business days, you will be entitled to a refund of all monies paid. You may also cancel this contract if this establishment goes out of business or relocates and fails to provide comparable alternate facilities within five driving miles of the location designated in this contract. You may also cancel this contract if you become physically unable to use a substantial portion of CrossFit MVA's services for 30 or more consecutive days, and your estate may cancel in the event of your death. You must prove you are unable to use a substantial portion of the services by a doctor's, physician's assistant or nurse practitioner's certificate, and the health club may also require that you submit to a physical examination, within 30 days of the notice of cancellation, by a doctor, physician's assistant, or nurse practitioner agreeable to you and this establishment (cost to be borne by CrossFit MVA). If you cancel after the three business days, the health club may retain or collect a portion of the contract price equal to the proportionate value of the services or use of facilities you have already received. Any refund due to you shall be paid within 30 days of the effective date of cancellation.
NOTICE
ANY HOLDER OF THIS CONTRACT IS SUBJECT TO ALL CLAIMS AND DEFENSES WHICH THE DEBTOR COULD ASSERT AGAINST THE SELER OF GOODS OR SERVICES OBTAINED WITH THE PROCEEDS HEREOF. RECOVERY HEREUNDER BY THE DEBTOR SHALL NOT EXCEED AMOUNTS PAID BY THE DEBTOR HEREUNDER.
COMPLAINTS
Should there be any complaints about CrossFit MVA, the purchaser is encouraged to resolve it internally. The Virginia Department of Agriculture and Consumer Services regulates health clubs in the Commonwealth pursuant to the provisions of the Virginia Health Club Act.
